The following is a list of Dr. Lev's procedures and fees. Dr. Lev's fees include Anesthesiologist's fee.

For overnight stay in Hospital, as indicated by an "(H)" there will be an approximate $2500 - $3200 charge in addition to the doctor's fee depending on how long your particular surgery takes. The price is a combination of the time spent in the O.R. plus around $200 for the actual overnight stay in a private room.

For outpatient procedures, as indicated by "(O.P.)" the charge for use of the operating facility and recovery room will be from $800 - $1500 in addition to the doctor's fee also dependent on how long the O.R. is used for your procedure.The Hospital Cima ONLY accepts credit cards as the method of payment. They DO NOT accept traveller's checks, bank or cashier's checks or cash.

An optional private duty English speaking nurse (12 hour overnight shift) is available for an additional $150 which will be included in your hospital bill. You must make sure to "request" a private duty nurse upon your registration at the hospital the morning of your surgery as a procedural requirement, but your private duty nurse will have already been booked for you in advance.

You must request the nurse at the same time that you book your surgery date. Please pay Dr. Lev with cash, traveller's checks or a bank check made out to Dr. Alejandro Lev.

· Dr. Lev uses the original Botox product imported from the U.S. made by Allergan.
